Social Media Case featuring Dr. Joseph Sarkissian
“Restoration of a ZERAMEX Zirconia metal-free implant that has been healing under the gums for 5 months.
My protocol here is in reverse and I suggest thinking twice before attempting it. The issue is that the implant is located slightly facial and has absolutely no keratinized gingiva facially.
Therefore the waterlase is used to create a trapezoid flap extending onto the palatal of the implant and transporting it over to the facial. Second, torque test the implant to ascertain integration. Third, placement of a zirconia 15 degree ANGLED abutment towards the palatal, fixed with a Vicarbo metal free screw at 25 Ncm torque. Fourth, suture the flap facial to the implant. Fifth, cover the screw access. Sixth, prep the abutment with a new fine diamond bur. Seventh, HERE is what you should carefully consider. In esthetic areas, let gums heal 3 weeks to their final position, prep the abutment at the new gum contours, then fabricate crown.
But in this case, after warning the patient that after healing, white zirconia may show at the gum line, we mutually agreed, based on his low lip line, to construct the crown today, as his gum line is not visible even when he smiles. The final crown will become his “healing abutment”. I had clear margins so I scanned with the cerec and finished the crown. Used just enough cement (dual cure relyx unicem) and made sure ALL cement was carefully removed. One final irrigation with ozonated saline into the flap and gave instructions not to eat on that side until follow-up appointment. Consideration: since there are no teeth on the same side, crown was slightly under-occluded.
A temporary partial denture will be made until the patient receives two more implants.“
About the Doctor
Dr. Joseph Sarkissian studied microbiology at the University of Alabama. He then attended the dental school of the University of Goettingen in the former West Germany, where he graduated in 1989.
Over the next eight years he practiced dentistry on the Mediterranean island of Cyprus. During that time he trained in homeopathy, completely abandoned the use of amalgam (mercury) fillings, and expanded his knowledge in the biological aspects of therapy.
In 1998, he moved to Los Angeles and received his license to practice in California. He currently owns a state-of-the-art biological dental practice in Glendale, California.
