Dr. Dean Licnblat: Placement of Zirconia Implant in a site with inadequate bone

Dr Dean Licenblat Social media case

What happens when there is a hole in our plans???

Complications are a reality for better or for worse. For better as they are learning experiences and for worse as we may not achieve what we set out to.

Without adequate lighting & magnification many perforations go unseen & consequently untreated.

This patient was referred for the placement of a zirconia implant in a site with inadequate bone. The patient has numerous allergies especially to metals so was keen on a zirconia solution.

The case was treated as follows:
1️⃣ Reflected a flap to allow access to the bone.
2️⃣ Drilled osteotomy to 1mm short of depth with versah drills according to osseodensification protocol.
3️⃣ 🕳 detected, teased the membrane up with hand instruments & sealed with 8×8 BioXclude from Snoasis.
4️⃣ Utilisation of Novabone putty to lift the sinus membrane ala Novabone Lift off
4️⃣ Insertion of a Zeramex XT RB ceramic fixture.
5️⃣ closure with 4/0 PTFE sutures.

What are your thoughts on small sinus perforation and how do you manage them???

ABOUT ZERAMEX® XT

ZERAMEX®XT has a tapered design allowing optimal primary stability. The internal connection has been specifically engineered to maximize the strength of rigid zirconia and eliminate any lateral forces on zirconia.

The precise fitting components offer variable placement close to bone level for enhanced restorative margin. The heart of the connection is the VICARBO® screw. Comprised of many individual threads of carbon fiber that run vertically, uncut down the length of the screw to durably absorb the tensile forces and give the VICARBO® screw its exceptionally high strength.
